Uno dei tratti più destabilizzanti delle sindromi PANS e PANDAS è la loro imprevedibilità. I sintomi non seguono un andamento lineare, ma si presentano in modo acuto, spesso improvviso, e possono fluttuare nel tempo. Questo rende difficile per le famiglie, gli insegnanti e persino i medici comprendere cosa stia realmente accadendo.
I bambini affetti da PANS/PANDAS possono manifestare un’ampia gamma di sintomi neuropsichiatrici e comportamentali. Tra i più comuni:
- Disturbo ossessivo-compulsivo (DOC)
- Tic motori e vocali
- Ansia intensa e generalizzata
- Alterazioni dell’umore (irritabilità, depressione, scatti d’ira)
- Regressione comportamentale e linguistica
- Difficoltà scolastiche improvvise
- Disturbi del sonno e dell’alimentazione
- Problemi di coordinazione e motricità
Questi sintomi possono comparire all’improvviso, spesso in seguito a un’infezione, e poi attenuarsi o riacutizzarsi nel tempo. Le famiglie parlano di “ondate”, di “giorni buoni e giorni impossibili”, di bambini che sembrano tornare se stessi per poi sprofondare di nuovo nel disagio.
La fluttuazione dei sintomi è una delle caratteristiche distintive di queste sindromi, ma anche una delle più difficili da spiegare agli altri. È importante che medici e insegnanti comprendano che non si tratta di capricci o instabilità emotiva, ma di un quadro clinico complesso che richiede attenzione, pazienza e competenza.
Riconoscere le fluttuazioni significa anche imparare a monitorare i sintomi, annotare i cambiamenti, individuare possibili fattori scatenanti (infezioni, stress, cambiamenti ambientali) e adattare gli interventi in modo flessibile. È un lavoro di squadra, che coinvolge la famiglia, la scuola e la rete sanitaria.
