Farmaco rivoluziona prevenzione. Salgono contagi ma taglio fondi
Solo due iniezioni l’anno per prevenire il contagio dal virus Hiv, invece delle attuali terapie preventive che, pur efficaci, prevedono la somministrazione di una compressa ogni giorno. Il nuovo farmaco, basato sulla molecola Lenacapavir, è stato appena approvato dell’ente statunitense per i medicinali, la Food and Drug Administration (Fda), e promette di contribuire a mettere fine all’epidemia di Aids su scala globale. Una svolta, secondo medici ed associazioni, che arriva in un momento in cui le malattie sessualmente trasmesse come l’Aids stanno rialzando pericolosamente la testa con un aumento dei casi soprattutto tra i giovani, ma che al contempo si scontra con la nuova politica di tagli dell’amministrazione Trump che proprio al settore della prevenzione dell’Hiv sta bloccando drasticamente i fondi.
La ‘duplice iniezione preventiva’ contro l’Hiv rappresenta oggi un passo avanti notevole, tanto che la rivista Science ha nominato il lenacapavir, prodotto da Gilead, “Scoperta dell’anno” del 2024. Si tratta appunto di una profilassi pre-esposizione (PrEP) per ridurre il rischio di Hiv sessualmente acquisito in adulti e adolescenti (di almeno 35 kg di peso). I dati mostrano che circa il 99,9% dei partecipanti che hanno ricevuto la terapia negli studi di Fase 3 Purpose 1 e Purpose 2 è rimasto Hiv negativo. Innovativo il meccanismo d’azione di lenacapavir rispetto alle altre classi di agenti antivirali attualmente approvate. Mentre la maggior parte degli antivirali agisce su una sola fase della replicazione virale, lenacapavir è progettato per inibire l’Hiv in più fasi del suo ciclo vitale e non presenta resistenza in vitro ad altre classi di farmaci esistenti. Il vantaggio maggiore sta appunto nella facilità di gestione del farmaco: oggi, l’aderenza alle terapie è infatti spesso un aspetto critico data la quotidianità con cui i farmaci Prep devono essere assunti.
L’obiettivo è ora rendere il farmaco disponibile anche fuori dagli Usa e nei Paesi in via di sviluppo, come quelli africani, dove l’Aids dilaga. “E’ enorme il potenziale impatto della nuova molecola. Potrebbe dare una svolta decisiva alla lotta alla diffusione dell’Hiv su scala globale”, afferma all’ANSA il presidente dell’Anlaids, Luca Butini. L’approvazione dell’Fda “è un’ottima notizia, soprattutto in ambiti di popolazione in cui la compliance al trattamento orale quotidiano o ‘on demand’ – spiega – sia scarsa e, dunque, la protezione verso l’infezione meno efficace”.
Secondo i dati dell’Oms, l’epidemia da Hiv ha causato circa 1,3 milioni di nuovi contagi e contribuito alla morte di 630.000 persone nel mondo nel 2023.
Ma i numeri spingono a tenere alta la guardia anche in Europa e in Italia. Secondo gli ultimi dati disponibili, sono 24.731 le nuove diagnosi di Hiv nel 2023 in Ue, un dato in aumento rispetto al passato. Tendenza confermata anche in Italia, che ne ha registrate 2.349 e che vede un incremento significativo dal 2020. In Italia le diagnosi di Aids sono 532, con un’incidenza di 0,9 casi per 100mila residenti, in aumento rispetto al 2022.
Ed anche se tra il 2010 e il 2023 le nuove infezioni da Hiv in Italia sono passate da 4051 a 2349, attestandosi su una riduzione del 42%, secondo gli esperti il dato è ben al di sotto delle aspettative. Inoltre, avvertono dal Telefono verde Aids e Ist dell’Istituto superiore di sanità, nonostante i giovani siano i più esposti, in molti casi sono disinformati, specie sulle modalità di trasmissione, tanto che vengono ancora poste domande sulla possibilità di essere contagiati con un bacio o una puntura di zanzara. L’Aids non va dunque sottovalutato, è il monito degli infettivologi. Ma resta la grande incognita degli effetti che potranno avere anche sul mondo della ricerca i tagli ordinati dal presidente Donald Trump al ‘Pepfar’, o ‘US President’s emergency plan for Aids relief’. Da gennaio, l’amministrazione Trump ha inferto duri colpi ai programmi di ricerca sull’Hiv e agli aiuti internazionali, con una brusca interruzione di gran parte dei finanziamenti da parte del governo statunitense. il taglio agli aiuti, si stima, potrebbe causare 11 milioni di nuove infezioni soprattutto in africa, ma non solo, entro il 2030.
